Responsibilities:
- Providing skilled nursing care to patients with advancing age, frailty, and serious illness.
- Performing patient assessments and developing treatment plans.
- Administering medications and treatments as prescribed.
- Collaborating with physicians, other nurses, and other healthcare professionals.
- Managing patient cases and coordinating care.
- Providing hospice care as needed.
- Maintaining accurate patient records and documentation.
- Communicating effectively with patients, families, and healthcare team members.
Required Skills & Certifications:
- Clinical assessment, medication administration, and treatment planning skills.
- Minimum two years of experience in hospital, home health, or hospice care.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Active Registered Nurse license in the state of employment.
- Bachelor's degree in Nursing or related field.
- Driver's license, car insurance, and personal vehicle for field work.
Preferred Skills & Certifications:
- Experience in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room settings.
